A malignant disseminated tuberculosis: concurrent intracranial tuberculosis with skipped multilevel spondylitis in a young immunocompetent patient

Background/aim. Mycobacterium tuberculosis (M. tuberculosis) infection can cause pulmonary and extrapulmonary (TB), resulted from hematogenous or lymphatic dissemination of the bacteria. Disseminated TB is characterized by presence two more non-contiguous sites spread. Dissemination to central nervous system (CNS TB) has several manifestations that devastating neurological complications. Several predisposing factors include older age, human immunodeficiency virus (HIV) coinfection, pharmacological immunosuppression. As manifestation CNS itself rare, diagnosing it remains a challenge. Prompt antituberculosis treatment needed improve patient’s outcome. This paper aims present rare case malignant disseminated in young immunocompetent patient. Case. presents an 17-year-old male with weakness four extremities. Through comprehensive history taking examination, patient was diagnosed TB, presenting as cerebellar tuberculoma, tuberculous meningitis, multiple skipped level spondylitis TB; concurrent TB. Then, underwent surgery for cervical lesion started on combination corticosteroid. After treatment, motoric sensoric functions improved he able urinate defecate normally. Conclusion. demonstrates importance considering differential diagnosis deficits, regardless fact immunocompetent. rapid initiation
